How do I create data-capture (non-scored) questions?

Overview

Data capture questions let you collect additional, non-scored information from evaluators during the evaluation process. You can use predefined response options, free-text answers, or numeric responses to gather structured data consistently across evaluations.

Getting there

Go to Settings > Quality Settings > click the Data Capture Questions tab

Here, you can create a list of questions that will be available when building your scorecards. For each question you create, you can also add a list of response options or enable a free-text response.

Creating a question

Click + New question to open the modal.

  1. Enter the phrase you want to use to prompt the evaluator.

  2. Decide whether evaluators can select multiple responses from a list of pre-populated options.

  3. Decide whether the question will use a free-text response or a list of pre-populated responses.

Pre-populated response list (Free text response = No)

You will be prompted to create a list of responses. When the evaluator answers the question, they will choose an option from the pre-populated list.

  1. Click + Add response to add your first response option

  2. Repeat the process until you have added the full list of responses

  3. Click Save changes

Tip: Creating a list of responses lets you filter results in reports based on a specific response.

Free-text response (Free text response = Yes)

No further action is needed. The prompt to add a response disappears and you simply click Save changes.

Tip: Free-text responses cannot be used to filter results. Because evaluators answer in their own words, the range of possible responses is too large to present as a list of filters.

Managing questions and responses

Once questions and responses have been added, they appear in the questions table. From here, you can:

  • Edit and delete questions while they remain unused (not yet answered on an evaluation). Once a question has been answered, it can no longer be edited or deleted to maintain reporting integrity.

  • Add new questions at any time.

  • Add new responses at any time.

  • Deactivate or reactivate questions and responses at any time by clicking the status pill to change it from Active to Inactive and vice versa. Only active questions and responses are available to evaluators.

Using questions on scorecards

If you add a question to a scorecard, evaluators using that scorecard will be prompted to answer the question at the end of the evaluation. The response is stamped on the evaluation and can be used (where available) to filter results in reports.

Response types

Data capture questions support four response types:

Evaluator enters a whole number (0 or higher, no decimals)

Important notes

  • Questions and responses used on evaluations become locked (shown with a lock icon) and cannot be edited or deleted

  • Deactivating a question hides it from future scorecards while preserving historical data

  • A warning icon appears when a question is active but all its responses are inactive
